Treatment Survey

The Form every Esthetician needs! The Treatment Survey asks clients to rate their last treatment, overall results, and experience so far. It allows you to check for new contraindications by asking about changes in medications and products, and recent sun exposure. This Survey also gives clients an opportunity to write down referrals! This package includes a download of the Treatment Survey as well as a video explaining its questions and how to utilize it in your own practice.
